Intake Forms
You should have recently received pre-visit paperwork requesting specific information. Please fill it out prior to our appointment so that I can be best prepared for our visit(s).
Consent for Care/HIPPA/Privacy Practices/Payment Policies
Please review all consent forms and notices and sign prior to your appointment.
Home Visits
I’ll be wearing a plain t’shirt with black pants, and a Sweet Relief LLC button for idenification.
The appointment will last approximately 1 hour.
Please have a private space available for assessment and discussion of your family’s history and needs, and put away any pets who live in your home. (I love dogs but they are not who I am coming to see!)
If possible and applicable, please have expressed breast milk or the supplement of your choice available.
Plan to have your pump available if you would like assistance with setup.
Let me know if you would like me to wear a mask, and if there are any cultural practices that I need to be aware of prior to entering your home.
Virtual Consults
Please set up in an area with privacy and limited noise and distractions for an uninterrupted virtual visit. I will send you a link ahead of time that you can use to access the visit at the time of the appointment.
Cancellation Policy
Remember that a $50 fee will be retained for cancellations within 24hrs of the selected appointment if not re-booked within the same week.
